stated no concern with the quality of care provided at Park Montessori. Based on interviews there was no indication of staff dragged a child care child into the bathroom. This agency has investigated the complaint alleging staff dragged a child care child into the bathroom. At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Director Hannah Park. Director was provided copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
