What the inspector wrote

On Thursday, October 2, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Director Selena Carrillo. The purpose of the inspection was to conduct an unannounced case management inspection for an Unusual Incident Report (UIR) received at Palmdale Regional office on 09/10/2025 regarding Child #1 (C1) falling and spraining wrist in the classroom. LPA Cha disclosed the purpose of the inspection to the Site Director. Upon arrival, LPA completed a safety inspection of the facility. LPA observed three staff providing care and supervision to 19 school-aged children. The current days and hours of operation are Mondays through Fridays; morning care from 7:00am-8:30am and afterschool care from 3:15pm-6:00pm. During the visit, LPA Cha made observations, interviewed staff, interviewed parents, and interviewed children. L PA Cha observed the carpet where C1 tripped. LPA did not observe any trip hazards on the floor. LPA observed carpet to be in good condition. LPA did not observe any other conditions that could have caused C1 to fall. Based on interviews, there was no concern about injuries or concerns about how staff handled incident. Based on observation and interviews, LPA deemed there is no indication of the incident being more than an isolated accident. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with Site Director Selena Carrillo .
