What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced 3 year required inspection to the above facility on 05/01/2025. LPA arrived at the facility at 1:36PM, identified self and met with Cathleen Trapani Director, who guided LPA on a tour of the facility. This is a school age program licensed for 96 school-age children and consist of 2 classrooms. The school age rooms are located on the 2nd floor of the licensed Preschool- Encino Presbyterian Children's Center #191201764. Hours of operation are Monday, Wednesday, Thursday, and Friday from 2:30pm to 6:00pm; Tuesdays from 1:30pm to 6:00pm. Summer program operates Monday-Friday 7:30 AM- 6:00 PM. All areas identified on the Facility Sketch were inspected. LPA toured the facility indoors and outdoors and a census was taken. There were no children present during today's inspection. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Tables and chairs meet the needs of the children in care. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There is a waiver on file for the outdoor yard. There are no pools or bodies of water. Page 1. Disinfectants, cleaning solutions and other hazardous items are inaccessible to children in care. All toilets and hand washing facilities are in safe and sanitary operating condition. Toilets used by school-age children provide individual privacy and are located outside of the classrooms at the end of the hall. Floors in the facility are clean and safe. Facility does not have access to a kitchen. Facility does not provide snacks for children, parents provide snacks. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. LPA observed two water dispensers at the facility. Children have their own water bottles, and the facility has disposable cups available. Facility has conducted water lead testing to comply with Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC\342\200\231s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test. LPA observed during file review, water was tested for lead in July 2023, LPA informed Director since they do not prepare food at the facility or provide drinking water from the faucets they do not have to test their water again. A sample of children\342\200\231s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report. Staff records were reviewed for Education, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, LIC503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of Page 2. $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
