Inspector’s narrative

What the inspector wrote

On 08/02/22, LPA reviewed children\342\200\231s records and observed that 12 of 12 Children files reviewed had immunization records in accordance with Title 22 Regulations. LPA received a copy of the enrollment agreement and it stipulates that \342\200\234All Schools, child care centers are required to enforce immunization requirements, maintain immunization records of children enrolled, and submit reports.\342\200\235 On 08/26/22, LPA conducted a site visit and observed 9 classrooms, 21 teachers, and 102 children in care. There were 2 teachers in each classroom and 3 floaters. The facility is open Monday through Friday from 8:30 AM - 3:00 PM; extended care in the morning is from 8-8:30 AM; and extended care on the afternoon is from 3-4:00 PM. The facility staff are reporting that when parents are late to pick up their children, they call the parents and stay with the children until they are picked up. Staff are reporting that they are not familiar with enrollment procedures and that administration deals with that. Administration is reporting that during enrollment, parents must complete the enrollment package which includes immunization records. Based on the evidence obtained, LPA was unable to corroborate that the Allegations, (1) Day care children are allowed to attend without required immunization's; and (2) Staff threaten they will leave children unattended. Therefore, the allegations are determined Unsubstantiated . A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegation occurred. A copy of this Report, Notice of Site Visit and Appeal Rights were explained and provided to the Director VEHANOUSH GABRIELIAN.
