What the inspector wrote
On January 9, 2026 at 11:00 am Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by Licensee\342\200\231s assistant, S2, who guided LPA on a tour of the facility. The licensee, Toni Dennis, arrived at the facility shortly after. Hours of operation are Monday \342\200\223 Friday from 7:00 am \342\200\223 5:30 pm. Per licensee, there are 14 children enrolled at the facility. LPA observed 3 infants and 9 preschool aged children in care and 3 assistants.
Family members residing in the home are 6 adults who obtain criminal record clearance. LPA explained to licensee that an updated Family Child Care Home application will need to be submitted to the department listing all the adults living in the home as well as their TB test. LPA verified all adults living in the home have current criminal record clearance.
This is a two-story home which consists of 5 bedrooms, 3.5 bathrooms, living room, family room, kitchen, dining room, downstairs basement, and back yard.
Off limits to children and parents include: 5 bedrooms, 2.5 bathrooms, living room, family room, kitchen.
Areas accessible to children in care are basement as a childcare space, the basement restroom, and backyard
At 11:30 am LPA was guided on a tour of the facility. Per licensee, parents enter through the side door of backyard which leads them to the entrance of the basement. Upon entrance to the basement, LPA observed multiple age-appropriate toys for children in care. Napping equipment was also observed in the form of cots and play yard.

The infant play yard did not have bumper pads or any hanging objects over or in the play yard. The infant play yard had an appropriate size mattress with fitted sheets. Per licensee, all bedding is taken home every week for washing purposes. LPA also observed a clean and sanitary changing table for children in care. The bathroom was then inspected in the basement area. The bathroom was clean and orderly. LPA did not observe any hazardous materials that can pose a danger to children in care. LPA then inspected the outdoor area and observed multiple toys of children in care. All areas were observed to be clean, orderly, comfortable, and well ventilated.
During tour LPA observed an infant swing/rocker in the basement that was accessible to children in care and a baby walker in the outdoor play area in a section that was supposed to be inaccessible. LPA did not observe infants on the swing/rocker or baby walker during the inspection. Per licensee, the equipment is not used for daycare children and will be removed. LPA observed S2 remove the infant swing/rocker from the basement and placed the child safety gate to prevent children from accessing a inaccessible area in the outdoor play yard. Per licensee, the infant swing/rocker and baby walker will be removed from the premises today.
There are first aid supplies available. Per licensee, the facility provides all meals for children in care. LPA reminded licensee that food brought from the children's homes, the container shall be labeled with the child's name and properly stored or refrigerated.
If a child becomes ill, the child is isolated in an area in the basement away from the other daycare children while the parents are called. A cot is available if needed.
Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children.
LPA observed a functioning smoke and carbon monoxide detector in the facility. LPA observed a 2A-10BC fire extinguisher in the kitchen area. The valve on the fire extinguisher pointed to the fully charged green section with a purchase receipt of 03/22/2024. Per licensee, the company who services the fire extinguisher has not. Licensee called the company during the inspection to schedule an appointment for service. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted and documented at least every 6 months. LPA observed the last emergency disaster drill documented on 11/28/2025.

Per the licensee, there are no weapons, bodies of water on the premises. LPA did observe a small dog in home. Licensee obtains the current Pediatric First Aid and CPR certification which expires on 05/02/2026.
LPA conducted a children file review and deemed it not complete. 1 out of 5 children were missing a signed Parent\342\200\231s rights (LIC 995), 1 out of 5 children were missing an Infant individual sleep plan (LIC 9227), 4 out of 5 children were missing immunization record for LPA to observe, 3 out of 5 children were missing 15 minute sleep logs. Per licensee, they were not aware they had to document infant safe sleep for infants in care. Licensee expressed that staff are always present and observing all children during nap time. LPA provided licensee a copy of all of the information that needs to be documents for infants safe sleep for children who are newborns to 24 months. LPA also advised licensee to read safe sleep regulations to ensure the facility is in compliance with title 22 regulations at all times
LPA conducted staff file review and deemed it not complete. 2 out of 4 staff could not provide proof that they have been immunized against influenza (or declination), pertussis, measles and TB, 4 out 4 staff could not provide proof of mandated reporter certificate.

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Earthquake preparedness Checklist.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, licensee, Toni Dennis confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA\342\200\231s observations and record review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety.
A notice of site visit was given and must remain posted for 30 days.
