What the inspector wrote

On Thursday, September 18, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Sharon Locke and Site Supervisor Irene Fernandez. The purpose of the visit was to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the Site Supervisors. LPA and Site Supervisors toured the facility. LPA observed 10 staff providing care and supervision to 32 infants. The current days and hours of operation are Mondays through Fridays from 7:30 a.m. to 6:00 p.m. The facility has a Toddler component. The facility also operates a day care license (#191231218). Physical Plant/Indoor/Children\342\200\231s Area/Bathroom: The facility operates on the grounds of Shepherd Church. The facility operates the infant program out of four classrooms referred to Classrooms 111,114, 116, and 118. The facility was inspected for safety, comfort, cleanliness, good repair, telephone service, heating, lighting, and ventilation. In Classroom 114 and 116, LPA Cha observed labeled cubbies for children's belongings. LPA observed refrigerator/freezer in working condition. Milk is disposed daily and bottles cleaned. Bottles are warmed using a crock pot. The diaper area was observed to be clean and sanitary with diapers and wipes. A separate sealed trash bin for diapers were observed. The portable sink was checked for working order. The cribs included only a tight-fitted sheet with no lose items. All cleaning products were stored out of reach of children. LPA observed age-appropriate soft toys and equipment. Cabinets accessible to children were locked or had a child safety lock. Feeding tables were cleaned after use and made of washable material. In Classroom 116, the napping area is located in a smaller room separated by a door. A large window allows visual observation when lights are on. LPA observed a staff member providing supervision to napping children in small room during time of inspection. The smaller napping room included cribs, cots, diaper changing area and a sink. In Classroom 118 and 111, LPA Cha observed sufficient napping cots for children, blankets, and linens. Cots were laid down in arm distance and did not block the door or walkways. There are two restrooms in classroom 118. LPA observed bathroom for safe and sanitary operating condition. Sinks and toilets in operable condition. Toilet paper, paper towels, and soap were available to children. LPA observed potty chair which were in clean and sanitary condition. Per Site Supervisor, there are no poisons on the premises. LPA Cha observed the furniture/equipment in good condition. LPA Cha observed no fireplaces and wall heaters. Kitchen: The off-limits kitchen is located in a separate room down the hallway from the classrooms. LPA observed the kitchen for safety, sanitation, and good repair. Kitchen consists of refrigerator/freezer, sinks, food preparation counters and pantries. The kitchen is shared with Shepherd Church. Knives and sharp items are safely stored, out of reach of children. Menus were observed and updated as needed. Children are provided with filtered water and use personal bottles which are labeled. Outdoor/Children\342\200\231s Play Area: LPA inspected the outdoor play space for safety, cleanliness, good repair, age-appropriate toys/equipment, cushioned floors, and shady areas. There is an emergency exit gate located in the infant playground, that does not open from the outside. Per Site Supervisor, staff check daily that emergency exit gate is closed before bringing children outside. Per record review, waiver on file for outdoor play space. Infants and Toddlers share play yard #1 at alternate times. Co-mingling is not permitted. Pools/Spa/Bodies of Water: Per LPA observation, there are no pools, spas, or bodies of water on the premises. File Review: LPA reviewed three children files which contained all required information. Safe sleep checks conducted via Bright Wheel app which is accessed by staff and parents. LPA reviewed Site Supervisor\342\200\231s file and one staff file. Per record review and interview, no staff member present had a current Pediatric CPR/FA certification on file. Per site supervisor statement, course is usually taken with the entire staff. Staff CPR/FA expired on 08/2025. Per site supervisor, staff is scheduled to take course. LPA informed site supervisor that at least one staff member present must have a current Pediatric CPR/FA on file, to be renewed every two years. A Type B citation issued. Postings: LPA observed required postings as follows: Facility License, Personal Rights (LIC613A), Parent\342\200\231s Rights Poster, Passenger Car Seat Law (PUB 269), and Emergency Disaster Plan. Other: Per Site Supervisor, smoking is prohibited on the premises. Per Site Supervisor, there are no firearms/weapons stored on premises. Per recorded documentation, Disaster drills last completed on 08/2025. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During this investigation, one Type B citation issued. See 809-D. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was printed and reviewed with the Site Supervisor Sharon Locke along with Appeal Rights.
