What Happened
Annual inspection discovered critical compliance violation: co-licensee Max Parra was residing in licensed facility despite outstanding 2021 removal letter from CPMB (Care Provider Management Bureau). Multiple physical safety deficiencies identified including hazardous items in reach of children, improper infant sleeping equipment, and fire extinguisher maintenance gaps. Licensee immediately committed to removing Max Parra from facility pending resolution.
Observations
- Co-licensee Max Parra residing in licensed facility despite April 26, 2021 CPMB removal letter—critical violation discovered during inspection
- Lysol spray observed on top of bookcase within arm's reach of children during nap time (removed during visit)
- Free weights and ladder in outdoor play area accessible to children (advised for removal)
- Fire extinguishers lack service tags and were last purchased 4/28/2021 (not serviced within required timeframe)
- Child observed sleeping on bouncer—prohibited equipment in family child care home
- No documentation of 15-minute infant sleep checks for 2 infants in care
- Licensee and one staff member missing tuberculosis clearance records
