What the inspector wrote

Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case management deficiency Inspection to the above facility. Upon arrival LPA met with the Facility Representative Rosalina Lukban , who guided LPA on a tour of the facility . During the visit LPA observed 46 children with 7 staff members. Based on the information obtained from the facility representative, Rosalina Lukban facility was unable to locate child`s medication to return it to the parent. Per facility representative, parents were notified that medication was lost. The parents confirmed that the medication was submitted to the facility and was never returned with child`s belongings. The facility representative reported that the parents had provided medication for emergency use only and it was stored in the classroom medication box. According to the representative , medication was moved to another classroom during the child`s transition . However, staff could not located it. Additionally, during that time the classroom teacher was on vacation and not aware of medication location. Based on the information obtained the following deficiency listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22..Deficiency that are being cited need to be cleared to protect the children’s health & safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and a copy of this report was provided and explained to Facility Representative, Rosalina Lukban along with Appeal Rights on 11/25/2025. End of the report.
