On 01/26/26, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced case management inspection due to the incident that occurred on Tuesday 01/20/2026. Upon arrival, LPA met with Rosalie Lukban, Director. During this inspection, LPA observed 53 children with 8 staff members. The Unusual Incident/ Injury Report was reported by the facility as required. During the inspection, LPA interviewed the staff member who witnessed the incident, as well as children involved. LPA obtained the ouch report, sign-in sheet from the date of the incident and written communication between facility and a parent. According to staff member it was the time for the children to line up when Child#1(C1) was observed laying on the carpet with their arms folded. Child#2(C2) attempted to assist C1 by pulling on her arm to help her stand. As reported by the staff member, C1`s arm twisted and appeared to dislocate. This information was confirmed by both C1 and C2. According to the staff member, first aid was provided immediately, and the child`s parents were notified. Per Director, staff spoke with the children regarding what occurred and ways to prevent similar incidents from occurring in the future. At this time, there is no preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when this incident occurred. Therefore, there is no deficiency being cited. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
