Incident investigation·March 2, 2026

On 03/02/2026, at 8:45am , Licensing Program Analyst (LPA) Shushanik Safaryan conducted a case management inspection due to the facility notifying Department regarding the incident of a child who required medical attention. LPA met with Facility Representative Rosalina Lukban , Director who guided LPA on a tour of the facility. During the tour, LPA observed 59 children with 9 staff members. The facility had reported the incident via email by submitting an Unusual Incident Report (UIR) on 02/10/2026. The incident was reported to the Department as required. The incident occurred on 02/10/2026 at 9:29 am, when Staff 1(S1) observed Child #1 (C1) having seizure. S1 called for help and dialed 911. C1 parents were notified and arrived before paramedics came. Child returned to school on 02/12/2026. During today’s visit, LPA interviewed director and staff member regarding the incident, reviewed child’s file , talked to the C1`s parent. Based on the information obtained C1 did not have any prior history of seizure or prescription medication. During the visit , LPA obtained sign in sheets from the date of the incident, copy of accident report, copy of discharge form. Per Director, parents will conduct follow up visit with doctor this week , which was confirmed by the parent. LPA discussed with the director to follow up with the parents and update health related forms for C1 as needed. Incidental Medical Services (IMS) policy was discussed.
