What the inspector wrote

On Thursday, October 9, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Gal Kessler-Rohs. The purpose of the visit was to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the Site Supervisor and toured the facility. LPA observed two teachers and five teacher assistant\342\200\231s providing care and supervision to 21 preschool-aged children. The current days and hours of operation are Mondays through Fridays 6:30am-5:30pm. The facility also operates an Infant program license (#191232472). Physical Plant/Indoor/Children\342\200\231s Area: The facility operates adjacent to the campus of Beachy Elementary School. The facility has a separate entrance gate, maintained locked with camera and alarm system installed. The facility operates the preschool program out of one classroom referred to as the Preschool Room. The facility was inspected for safety, comfort, cleanliness, good repair, telephone service, heating, lighting, and ventilation. LPA Cha observed labeled cubbies for children's belongings. LPA observed the diaper changing area which was clean and sanitary with diapers and wipes. A separated sealed trash bin is used for soiled diapers. The sink was located within arm distance from changing tables. All cleaning products were stored in high shelves, out of reach of children. LPA observed age-appropriate toys/equipment, books and supplies. Children are provided cots which are spread out within arm distance and do not obstruct walkways. Children use blankets and linens which are stored in individualized bins. An isolation area is provided in the main office. LPA observed no fireplaces and no wall heaters. Child Care Bathroom: LPA observed one child care bathroom (located on the back left from front door) for safe and sanitary operating condition with no hazardous materials. There were two toilets and four sinks. Toilet paper, paper towels, and soap were available to children. Kitchen: The kitchen is located in separate infant classroom. LPA observed the kitchen for safety, sanitation, and good repair. Kitchen consists of refrigerator/freezer, sink, food preparation counters and pantries. Knives and sharp items are stored in cabinet, made inaccessible by an automatic lockable screen door. The facility participates in a food program. Menus were observed and updated as needed. Children are provided with filtered water and use personal bottles which are labeled. Outdoor/Children\342\200\231s Play Area: LPA inspected the outdoor play space for safety, cleanliness, good repair, age-appropriate toys/equipment, cushioned floors, and shady areas. The outdoor play area is enclosed with fencing. LPA observed a anchored play structures with cushioned flooring for safe falls. The outdoor play space is shared with the preschool program. Per Site Supervisor, children do not co-mingle and outdoor play times are staggered. Per LPA observation, there are no pools, spas, or bodies of water on the premises. File Review: LPA reviewed two children files which contained all required information. LPA reviewed Site Supervisor\342\200\231s file and four staff files which included all required information. LPA reviewed the educational records of all staff present to verify that facility was within ratio requirements. Site Supervisor CPR/FA expires on 06/2026. Per record review, Staff #1 (S1) Mandated Reporter training expired on 09/2025. LPA reviewed three other staff files which included current Mandated Reporter training. LPA informed Site Supervisor that Mandated Reporter training must be completed every two years. Site Supervisor will send proof of S1 completion of Mandated Reporter training to LPA Cha via email by 10/16/2025. A Technical Violation issued. Postings: LPA observed required postings as follows: Facility License, Personal Rights (LIC613A), Parent\342\200\231s Rights Poster, Passenger Car Seat Law (PUB 269), and Emergency Disaster Plan. Other: Per Site Supervisor, smoking is prohibited on the premises. Per Site Supervisor, there are no firearms/weapons stored on premises. Per recorded documentation, Disaster drills last completed on 10/2025. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Per record review, last lead testing conducted on 07/29/2023. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During this investigation, one Technical Violation issued. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was printed and reviewed with the Site Supervisor Gal Kessler-Rohs along with Appeal Rights.
