What the inspector wrote

On Thursday, May 14, 2026, Licensing Program Analyst (LPA) Evelyn Garcia conducted a case management-incident inspection in regards self-reporting incident. LPA met with director Rocio Gomez. LPA stated the purpose of the inspection was to follow-up on a self-reported Unusual Incident Report (UIR) that occurred on April 27, 2026, at the center where a child remained on the playground with no active supervision during an outdoor transition time. LPA Garcia toured the facility, verified a census of 56 children and 11 teachers providing care and supervision. During this investigation LPA Garcia conducted an inspection of the facility where the incident occurred and conducted confidential interviews with children and staff members. Based on interviews conducted, it was revealed that child #1 was left unsupervised on the playground between 3-5 minutes. LPA informed director due to the child remaining on the playground with no active supervision, it is a violation of California Code of Regulation, Title 22 101229 (a)(1) Responsibility for Providing Care and Supervision. Based on the evidence gathered, the facility has been cited with a Type B violation. Please see attached LIC 809-D for citation. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted, and report was reviewed with director Rocio Gomez.
