What the inspector wrote

On 07/16/2025, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Case Management - Incident visit due to an incident that occurred at the facility. LPA arrived at the facility at 8:30 AM and met with Lena Blakeney, Director who guided LPA on a tour of the facility. There were 49 children in care and 17 staff present. The incident that occurred on 07/01/2025, was reported to the Department on 07/03/2025, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Parent #1 and Parent #2 reported that Child #1 and Child #2 engaged in an activity that was not appropriate. LPA conducted interview with Director, interviews with staff and obtained documentation for this incident. Based on information LPA received from interviews, there were no deficiencies cited for the incident that occurred on 07/01/2025. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Lena Blakeney, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
