What the inspector wrote

On 3/18/26 LPA Jeanine Lipsey met with Director Lena Blackeney for the purpose of an unannounced Case Management - Deficiencies visit. Upon entry, LPA Lipsey observed 37 children, being supervised by eleven staff members. Classroom #120 has 7 children with 3 staff Classroom #130 has 7 children with 2 staff Classroom #140 has 6 children with 2 staff Classroom #150 has 9 children with 2 staff Classroom #160 has 8 children with 2 staff Upon review of the guardian roster, teacher Lisa Garcia was found to have been working, residing or volunteering at the facility without a fingerprint clearance transfer. Date of hire was 7/17/25. Based on today's observation, today's violation  will be assessed a civil penalty. See attached LIC 809D. A Civil Penalty of $500 was assessed due this violation.  See LIC 809D On 3/5/26 LPA Lipsey was informed that Director Lena Blackeney was the new Director and failed to report to the department. Hire date was 8/29/22. Exit interview conducted and report was reviewed with Director Lena Blackeney on 3/18/26. A copy of this report and a Notice of Site Visit was given and must remain posted for 30 days.
