What the inspector wrote

On 4/1/26 at 12:50PM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced, in-person Case management incident inspection.  The incident was reported to Community Care Licensing on 3/23/26 via phone.  The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident occurred on 3/19/26. Incident: A substitute teacher pressed their finger into a child's arm leaving a mark/scratch. Upon arrival, LPA was guided on a tour of the facility by Director Lena Blackeney to whom the purpose of the inspection was announced. Census:  There were forty-six children being supervised by thirteen staff. Classroom #120 for 18 months- 2.5-year-olds 9 children with 3 staff Classroom 130 for 24–36-month-olds 10 children with 2 staff, Classroom #140 for 24–36-year-olds 8 children with 3 staff Classroom #150 for 3–5-year-olds 8 children with 2 staff, Classroom #160 3–5-year-olds 11 children with 3 staff. During the inspection, LPA obtained the substitute teacher's contact information and conducted interviews with 2 staff. This incident needs further information and will be concluded at a later date. Exit interview conducted and report was reviewed with Director Lena Blackeney .  A Notice of Site Visit was given and must remain posted for 30 days.
