On October 8, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPAs met with director, Eloise Morin who guided LPA on a tour of the facility. LPA observe 52 children in care with 13 staff members the time of inspection. The purpose of the inspection is to follow up on an incident that occurred on 09/17/2025 and was reported to the department on 09/18/2025; the incident was reported in a timely manner. During the inspection LPAs obtained a copy of the facility roster. LPA interviewed Staff #1 (S1) prior to the inspection. LPA interviewed Child #1 (C1) and LPA attempted to interview Parent #1 (P1) but there was no answer. Based on interviews, S1 stated the classroom was playing down at fellowship hall due to the weather being too hot. Per S1, there were a total of 13 children with S1 and another staff member (13:2). Per S1, they were assisting another child and the other staff member had taken some children to the restroom. S1 stated, that at the time they were helping another child, they heard C1 cry. Per S1, they asked C1 what happened and C1 stated they hit the pillar. S1 stated, they took C1 to the restroom to clean the injured area, brought C1 to the office, applied an ice pack and informed parents. C1 stated that in fellowship hall they were running and hit their head the pillar. C1 showed LPA where they hurt themselves. Per C1, S1 washed off the injured area and gave C1 band aid. The facility is not being cited any deficiencies at this time. An exit interview was conducted and a copy of this report was provided along with Notice of Site Visit. The Notice of Site Visit must be posted for 30 days.
