On November 10, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with Eloise Morin, Campus Director who guided LPA on a tour of the facility. LPA observed 52 children with 13 staff members. The purpose of the inspection is to follow up on an incident report that occurred on 10/21/2025 and was reported to the department on 10/22/2025. The incident was reported timely to the department. During the inspection LPA interviewed Staff #1 (S1) and Child #1 (C1), LPA observed the area where the incident occurred and obtained other relevant documentation. Based in interviews S1 stated, C1 was running in the sandbox area and when C1 was coming out of the sandbox area, C1 tripped hitting their lip on the concrete. LPA observed the sandbox area to have a small concrete barriers that surround the sandbox. Per S1, they were approximately 5 feet away from C1 when the incident occurred. S1 stated they attended to C1 and provided care to the lip and provided an ice pack. Per S1, they notified Parent #1 (P1) of the incident. Per S1, C1 was taken to seek medical care where stitches were applied. C1 stated they were running in the sandbox area where they tripped and hit their lip. C1 stated they were taken to the doctor. The facility is not being cited any deficiencies at this time as the incident appears to be an accident that occurred quickly and was unable to be prevented. An exit interview was conducted and a copy of this report was provided to director along with Notice of Site Visit. Notice of Site Visit must be posted for 30 days.
