On September 25th 2025 Licensing Program Analysts (LPAs) Mariah Aguirre and Stephanie Li conducted a case management inspection due to an incident that was reported to the Department on 09/03/2025. The incident took place on 08/29/2025 but was reported by parent to the facility on 09/02/2025 and was reported to the Department in a timely manner. LPAs met with Site Supervisor Patricia Yaghnam and lead teacher Fong Young, who guided LPAs on a tour of the facility to obtain the census of 31 children with 5 staff. LPAs conducted interviews with students and spoke with the teacher #1 regarding the incident. The facility had self-reported the incident to the Department on 09/03/2025. Unusual Incident Report (UIR) was received via phone call. The incident reported consisted of child #1 stating to their mother that teacher #1 hit C1's hand three times.The facility reported the incident within the required 24-hour time frame. LPAs could not conduct an interview with child #1 as they are no longer attending the site the incident occurred at. LPAs conducted an interview with child #2. Child #3 was not verbally responsive and interview could not be conducted. Child #4 declined to be interviewed. LPAs interviewed teacher #1 1 of 2 At this time, the incident involving C1 and T1 require further investigation. An exit interview was conducted with Site Supervisor Patricia Yaghnam and a notice of site visit was given. LPAs informed site supervisor it must remain posted for 30 days. A copy of this report along with appeal rights were given to Site Supervisor during the exit interview. 2 of 2
