What the inspector wrote

On November 20th 2025 Licensing Program Analyst (LPA) Mariah Aguirre conducted a case management inspection due to an incident that occurred at the facility on 09/02/2025. LPA met with Lead Teacher Fong Young who guided LPA on a tour of the facility to obtain the census of 13 children and 3 staff. The facility had self reported the incident to the Department on 09/03/2025. Unusual Incident Report (UIR) was received via phone call. The UIR consisted of a possible personal rights violation. During the inspection, LPAs conducted interviews with Staff #1 (S1), Children #1, #2, #3, and #4 (C1, C2, C3, C4) and LPA attempted to interview Parent #1 (P1), but was unsuccessful. LPA left a voicemail for P1 stating the purpose of the call and provided a call back number but did not receive a response. A current children's roster was obtained. The incident consisted of C4 telling P1, S1 slapped them on their hand three times while at the facility on 08/29/2025. Per UIR, P1 disclosed that there were no marks on C4 when picking up from school. During interviews conducted, C4 stated that S1 did not slap their hand three times while at the facility. Children #1, #2, and #3 made no disclosures. Based on interviews conducted, there are no deficiencies being cited at this time. An exit interview was conducted with Lead Teacher Fong Young. A notice of site visit was given and must be posted for 30 days. A copy of this report along with appeal rights were issued to Lead Teacher Fong Young.
