Licensing Program Analyst (LPA) Alicia Mooberry conducted a case management to the facility to follow up on incident report that was reported on an incident report that occurred on 12/16/25. LPA met with Facility Representative Amy Nalasa, Director, who allowed entry into the facility. There were 22 children 7 staff supervising. On 1/13/26, the Director reported and incident regarding Child #1. Per director, on 12/16/25 child #1 reported to staff #1 an incident that occurred at the home. The incident was reported to the authorized representative on the same day. The incident was not reported to the required agency and reported to the Licensing Office on 1/13/26. The director was reminded of the requirement to report any unusual incident to the department within 24 hours of occurrence. LPA received the completed written report (LIC 624) on 1/13/25. LPA discussed with Director, Mandated Reporting requirements by all staff at the facility and reviewed LIC 9108 Statement Acknowledging Requirement to Report Child Abuse. 1 B deficiency is cited during this inspection per Title 22 Regulations 101212(d)(1)(D) . Exit interview was conducted with Director Amy Nalasa. Appeal Rights were discussed and provided. Notice of Site Visit was provided and needs to be posted in public location for 10 Days.
