Correction check·August 29, 2025

On Friday, August 29, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted a Plan of Correction (POC) visit to verify the corrections of previously cited Type A deficiencies issued under title 22 regulations. Deficiencies issued during an inspection conducted on July 30, 2025, (one) Type A deficiency cited under Section 101161 \342\200\223 Limitations on Capacity and (one) Type A deficiency cited under Section 1596.871- Criminal Record Clearances. Deficiencies issued during an inspection conducted on August 1, 2025, (one) Type A deficiency cited under Section 101161 \342\200\223 Limitations on Capacity and additionally a deficiency issued during an inspection conducted on August 5, 2025 (one) Type A deficiency cited under Section 101161 \342\200\223 Limitations on Capacity. Census was taken and LPA Perry observed 25 children in care. During the inspection LPA Perry that the summer camp program, was not operating on the premises during today\342\200\231s visit. LPA did not observe any children being dropped off at the facility for the summer camp program. Facility Director confirmed that the summer camp program ended on August 8, 2025. LPA verified that Staff #1 was cleared on 8/1/2025, Staff #2 was cleared on 8/2/2025 have been fingerprint-cleared and associated with the facility. Per assistant director Staff #3 is no longer employed with facility. LPA Perry verified that these deficiencies were subsequently cleared. A Letter of Deficiency Citations Cleared was provided to the facility. Per regulations, the letter must be kept on file for three years and made accessible to the public upon request. An exit interview was conducted with Assistant Diretcor Karen Zimbehl. Appeal rights were provided. LIC 9213 \342\200\223 Notice of Site Visit was issued and is required to be posted for 30 days. All Type A cited deficiencies have been corrected. No citations issued today; Facility is in compliance.
