What the inspector wrote

On Friday, January 9, 2026, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection- Incident follow-up visit regarding an unusual incident that was self reported to the Department on December 4, 2025. Upon arrival, LPA Perry met with the facility director, Pam, who provided guided LPA Perry on a tour of the facility. Census was taken. During the investigation, LPA Perry conducted interviews with staff, children, and authorized representatives of C #1 and C#2. P #1 reported that C #1 disclosed that another C#2 had touched C#1 in the private area and that the concern was immediately reported to the facility, followed by a meeting with the director the next day. LPA reviewed the children\342\200\231s roster and children\342\200\231s identification records and emergency information (LIC701) to verify that both C#1 and C#2 were enrolled at the facility during the timeframe of the alleged incident. LPA also obtained and reviewed other pertinent documents as well as facilities internal follow-up investigation. Child interviews did not disclose any information to the allegation on the unusual incident report. . Staff interviews were conducted, and no staff reported witnessing or having knowledge of the alleged incident or any such behaviors of that nature. Staff stated that an internal investigation was conducted. It was also noted that C#1 has since dis-enrolled from the facility. Based on information obtained through interviews and document review, there was insufficient evidence to substantiate the unusual Incident allegation reported to the Department. The facility is in compliance with Title 22. No deficiencies were cited at this time. An exit interview was conducted with the facility director Pam. A copy of this report and appeal rights were provided. The report must be posted for public review for a period of 30 days.
