What the inspector wrote

On 7/14/2022, Licensing Program Analysts (LPAs) Carolyn Tuba and Monique J. Ayala conducted an unannounced case management inspection. The purpose of this visit was to follow-up on an incident that was filed with the Department on May 26, 2022; The incident was reported timely to the Department. Upon arrival LPAs met with Director, Kori Vartanian who guided LPAs on a tour of the facility. LPAs observed 14 children in care with 3 staff. Description of incident: On May 26, 2022 Child 1 fell on the ground in the classroom during music and movement. While using the restroom child 1 told staff 2 that his private area hurt. Child 1 was observed to have been injured when blood was on his private area. There was no blood observed in Child's 1 underwear. Parent 1 was contacted, Parent 1 took child 1 out of the facility. The injury was observed to have not been severe enough to require any first aid. During this inspection LPAs interviewed Director, Staff 1 and Parent 1. LPAs also obtained a copy of the Facility Roster, a copy of the 2021-2022 Preschool Schedule, a copy of Injury Report given to Parent 1, obtained an accurate count of children present on the day of the incident and LPAs took photo of the area the injury occurred. LPAs were unable to interview children due to children not being present today that were present on the day of the incident. Based on LPAs observation, record review and interviews conducted, the incident did not occur due to lack of supervision as Staff 1 was located in the middle of the classroom and observed the incident that occured. During the time of the incident there were approximately 9 children with 3 staff members. The incident occurred at 3:10pm and per Parent 1, Parent 1 was notified of the incident at 3:15pm. The facility is found to be in compliance with Title 22 Regulations and no deficiencies are being cited. The director was reminded to continue to report any unusual incidents to the department. An exit interview was conducted with director, Kori Vartanian a copy of the report was provided along with Notice of Site Visit and appeal rights .
