What the inspector wrote

On 07/11/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management visit. LPA met with Marcela Torres, Director and explained the purpose of the visit. The reason for today’s visit is to address an incident that occurred on 06/12/2025 and was reported to the department on 06/12/2025. The self-reported incident is regarding personal rights. During the course of the investigation, LPA Kruz Long obtained a copy of the children roster, staff statements, termination notice and interviewed Staff #1 (S1), Staff #2 (S2) in the office and attempted to interview Child #1 (C1). According to the interview with Staff #2(S1), who was present at the time of the incident, indicated that Staff #3(S3) threw a book towards Child #1(C1). The book did not hit C1 but child was startled. S2 reported the incident to the Director and Staff #1. An internal investigation was conducted and S3 was placed on administrative leave and later terminated from employment with the facility. Per S1, S3 admitted to throwing the book. Per S3’s written statement, S3 stated that the book was thrown. Based on interviews conducted and record review, the deficiency was cited in accordance with the Title 22 of the California Code of Regulations and Health & Safety Codes. See LIC809D page for documentation of deficiency. LPA advised Director that an office meeting will be requested between Maryvale Day Care Center and Community Care Licensing. An exit Interview was conducted and a copy of this report and appeal rights along with Notice of Site visit was provided to the Director.
