What the inspector wrote

On 08/25/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management visit. LPA met with Marcela Torres, Director and explained the purpose of the visit. There are currently 51 Children enrolled. 36 Children and 10 Staff are present in 4 different classrooms during today's visit. The reason for today’s visit is to investigate an incident that occurred on 08/13/2025 and was reported to the department on 08/13/2025. The self-reported incident is regarding personal rights. During the course of the investigation, LPA interviewed Staff #1 (S1) and Child #1 (C1) in the office. LPA also obtained and reviewed a copy of statements written by Staff #1 (S1) to Staff #5 (S5) and a termination letter. According to the interview with S1, S2 reported that C1 disclosed to them that S5 hit C1 on the hand. An internal investigation was conducted by the facility. Per interview and written statements by staff, there were no witnesses to this alleged personal rights incident. LPA reviewed written statements which verified they did not witness this alleged incident. S1 interviewed S5 who denied the allegation and LPA made several attempted to contact C5 but did not receive a response. LPA interviewed C1 who did not provide corroborating information to validate this allegation. Based on interviews and record review, there were no corroborating information to determine that there was a personal rights violation. The facility is not being cited any deficiencies today. An exit interview was conducted and a copy of this report and appeal rights was provided to the Director. A Notice of Site Visit was provided; Notice of Site Visit must be posted for 30 days.
