What the inspector wrote

On December 16, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- incident inspection at the above facility. LPA met with Interim Director, Victoria Casas who guided LPA on a tour of the facility. LPA observed 45 children with 9 staff. The purpose of the inspection is to follow up on an incident report that occurred on 12/03/2025 and was reported to the department on 12/04/2025. The incident was reported timely. During the inspection LPA interviewed Staff #1 (S1) to Staff #3 (S3), LPA interviewed Child #1 (C1) to Child #3 (C3), LPA attempted to interviewed Parent #1 (P1) and Parent #2 (P2) but there were no answers. LPA left a voicemail for P1 and P2 with LPAs contact information. LPA obtained copies of relevant documentation. Based on interviews conducted, S1-S3 stated that they did not observe any thing unusual occur with C1. Per S1, C1 was crying throughout the day asking for their mommy and S1 stated that they did some breathing exercises with C1. Per S1, once C1 stopped crying C1 continued with normal activities. Per S2, C1 usually cries asking for their mommy and daddy and staff reassures C1 that their parents are going to come soon and C1 continues with normal activities. Per S3, they did not observe C1 cry until after nap time but also stated there are times C1 wakes up and starts to cry because they miss mommy and daddy. Per S1-S3, they comfort C1 when they cry. Per S1, is was not until P1 arrived that C1 disclosed their arm hurt. Per S1-S3, C1 had not made any disclosures of their arm hurting throughout the day. Per C1 to C3, there were no disclosures of anything happening to C1 at the facility causing an injury to their arm. LPA attempted to contact P1-P2, there were no answers. At this time there are no deficiencies being given. An exit interview was conducted an a copy of this report was provided along with Notice of Site Visit that must be posted for 30 days.
