Incident investigation·February 11, 2026

On February 11, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Coralee McPherson who guided LPA on a tour of the facility. LPA observed 37 children in care with 12 staff members/working parents. The purpose of the inspection is to follow up on an incident that was reported to the facility on 02/02/2026. The actual date of the incident is unknown. The incident was reported to the department on 02/02/2026. During the inspection, LPA interviewed Child #1 (C1) - Child #2 (C2), Staff #1 (S1) to Staff #3 (S3) and obtained a current facility roster. Based on interviews with C1, C1 did not disclose any incidents occurred with C2. Per C2, they do not play C1 and did not disclose any incidents occurred. Per S2, they did not observe any incidents and C1 did not disclose that any incidents had occurred with C2. Per S1-S3, there is always visual supervision when children are utilizing the restrooms and did not observe any incidents with C1 and C2. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided and must be posted for 30 days.
