What the inspector wrote

Licensing program analyst Kamile Martin conducted an unannounced Case Management inspection due to an incident that occurred on 7/28/ 25 and was reported to the department on 7/28/25 via phone call. LPA met with Director Hilda Ontiveros -Koorn whom the reason for the visit was explained . Hilda guided LPA on a tour of the facility. There were 19 children present and 9 staff with 1 volunteer. The facility was observed to be operating within the license capacity limitatio ns. On 7/28/25 an unusual incident report was made to the Department regarding a child told her parents that a teacher pulled her hair and pinched her nose. Director Hilda took immediate action and began investigation action by interviewing staff as well as held a meeting with both parents and child, teaching staff attended the meeting as well. Ms Roshendi Fernando, Ms Elizabeth Lpoez , and Ms. Natalia Gonzalez. Teachers’ statements were written and attached to the UIR submitted. A statement was written on behalf of the director documenting the incident, the investigation and the follow up plan. Since the parents met with staff. Per parent the father wanted to understand the order of events and how they occurred mom read the statement and mom signed and father wanted a translated version in Spanish and once he received the translated version he signed the statement. Since then, the child has continued care and arrived to school without any concerns. During today’s visit, LPA interviewed the director Hilda Ontiveros Koorn, reviewed the children’s roster, tour of the preschool classroom, toddler classroom Infant classroom, outdoor playground and kitchen area and observed the children in care. Based on the information obtained during this investigation, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00 Exit interview conducted and report was reviewed with Hilda Ontiveros Koorn.
