What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Facility Representative Claudia Casillas who guided LPA on a tour of the facility. On 4/16/2025, it was reported to the department that child # 1 was playing on the apparatus and child # 2 informed teachers that child # 1 was bleeding, no staff witnessed the incident. Teacher's noticed a cut on chin. The incident was reported to the Department within the 24-hour period and the written report was received via email on 4/17/2025.. During today\342\200\231s inspection staff interviews were conducted and documentation was reviewed. Based on interviews Child # 1 fell somewhere on the structure, hitting their chin causing a cut that resulted in needing stitches. Child # 2 informed staff that child # 1 was bleeding, Interviews disclosed that incident was not observed by any staff and there was no direct supervision on structure. Based on the LPA\342\200\231s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety.
