What the inspector wrote

Licensing Program Analysts (LPAs) Nolan Tcheng and Stephanie Li conducted an unannounced 3-Yr random inspection to the above facility. At 11:11am, LPAs met with licensee Stella Rios. Also present during this inspection Licensee\342\200\231s Assistant. The licensee states that they currently has 11 children enrolled. A current children\342\200\231s roster is available and is current. Licensee\342\200\231s hours of operation are 6am-7pm, Monday-Friday. This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, front yard and backyard (fenced). The children use the living room, dining room, kitchen, and one bathroom. Per licensee, areas off limits to children and parents include: all bedrooms, all bedrooms, one bathroom, front yard, and back yard. All areas identified on the facility sketch were inspected. The licensee provides food for children in care. The licensee states that 3 adults and 0 children currently live in the home. Persons residing in home are identified on attached LIC811. Licensee states that they currently have one assistant. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no weapons, firearms or bodies of water on the premises. At 11:20am, LPA was led on a tour of the facility by the licensee. Upon entry into the home, LPAs reviewed the living room, which is being used as the main child care area. Per licensee, the side of the living room will be used as an area for an ill child until an authorized representative comes to pick up child. Emergency Disaster Plan, Parent\342\200\231s Rights Poster and the Facility License are observed to be posted. The tour continued into the dining room which licensee states they are using for snacks and homework time. Licensee is currently caring for school age children. The kitchen was reviewed next which was free of any hazards. The tour continued to the back of the home where the bathroom for children in care was. Detergents, cleaning compounds, medications, and other items which could pose a danger are inaccessible to children. REPORT CONTINUES PAGE 1 of 3 At 11:25am, a fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged and did not have documentation of last service date. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. LPAs provided an Advisory note regarding the issue. There are first aid supplies available. Smoke and carbon monoxide detectors were tested and are operable. Licensee states they use both the cell phone and the landline. At 11:35am, LPAs observed the back yard area. The outdoor play area was observed to be fenced. At this time, children are not using the backyard due to some maintenance work done on the house. Per licensee, the backyard will be available to kids in the future. The licensee is observed to be operating within the license capacity limitations. The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expired on 03/2019. Licensee states that they have already made appointment for new training. Licensee and other personnel have complete mandated reporter training that expired 03/14/2021 . LPAs discussed with licensee of the requirement to complete the Mandated Reporter training every two years. The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles. Children\342\200\231s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information speci\357\254\201ed by regulation. The following was discussed : - There is one small dog on the premises. Dog stays in the off limits areas when children are present. -Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family child care facility. -Smoking is prohibited in a license family child care home. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated REPORT CONTINUES PAGE 2 of 3 Incidental Medical Services (IMS) policy was discussed. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA\342\200\231s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety A notice of site visit was given and must remain posted for 30 days.
