Follow-up on corrections·December 4, 2025

While conducting an investigation for a complaint, Licensing Program Analyst (LPA), T. Tran observed the following deficiency: On 10/03/25, during the interview with the facility staff, it was confirmed that when staff change children in a standing position in the children's restroom, and when a child is heavily soiled, the child was taken to the nursery room and changed on a cot. Based on the facility sketch, preschool children do not have access to the Nursery room. Nursery room is limit to Sunday School. On 12/4/25, LPA observed the facility had installed a changing table in the children's restroom in room 102. Staff will no longer needed to take children in the Nursery room for diaper change. Facility was cited a type B deficiency. See Facility Evaluation Report LIC 809D for deficiency cited. Citation was cleared during today's visit. A copy of this report was provided to the licensee and an exit interview was conducted.
