What the inspector wrote

Information from the complainant indicates that the facility is withholding food from children and that they do not notify authorized representatives of injuries sustained at the facility. Director states that children are a provided more food until there they are unable to serve any more. Some food, such as cheese sticks are purposely only served as one portion. If a child asks for food when it is not meal time, Director states that teachers will discuss with child when meal time will be. If they need to, they will go retrieve a snack for them or have them take something out of their lunch box. Director states injuries are first assessed, treated, and then an ouch report is made. Parents are provided the report during same day pick up from facility. During staff interviews, Staff #2 states that if a child asks for more food they will feed them. It was disclosed that families provide their own child’s lunch and that the facility provides AM and PM snack. If a child asks for something to eat when it it is not meal time, Staff #2 says they usually bring the child over to their lunch box to do it. Staff #3 and #4 stated they try to provide two servings of snack but could try to provide more if they notice that the child’s lunch was smaller than usual. Staff #4 reminds the children that if it is not meal time. All staff state they do not tell a child “No” if they request food. All Staff interviews disclosed that they create accident/ouch reports for children for all injuries. Parents are notified and asked to sign a copy of the report when they pick up the child. Injuries to the neck and up, require a phone call right away to the parent to notify them. During the inspection on 06/21/2022, LPA observed the ouch reports created for children, in file. All parents, during interview, explained the injury procedures that the facility follows. There were no disclosures regarding the allegations. Based upon the evidence as presented above, this agency has investigated the allegation above and has determined that the allegations are Unsubstantiated. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. No deficiencies are being cited for the allegations listed above The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
