What the inspector wrote

On April 15, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management inspection - incident at the above facility. LPA met with Sarin Assadvazadorian, Director who guided LPA on a tour of the facility. LPA observed 43 children in care with 8 staff members. The purpose of the inspection is to follow up on an incident report that occurred on 04/01/2026 and was reported to the department on 04/02/2026. During the inspection LPA interviewed Staff #1 (S1) and Child #1 (C1), LPA observed the area where the incident occurred, LPA obtained a copy of the internal incident report and a facility roster. Based on interviews, S1 stated that they observed C1 playing the bridge part of the climbing structure and observed C1's hand slip from the railing. Per S1, C1 had both hands on one side of the railing. Per S1, when C1 hands slip from the railing C1 was semi leaning over the railing and with the cause of C1's hands slipping C1 fell forward. Per S1, when C1 fell forward from spacing of the railing, C1 hit their head on the bridge part of the climbing structure prior to landing in the sandbox. Per S1, Staff #2 (S2) was closer to C1 and immediately assist C1 with first aid care (applied pressure with towels, an ice pack was provided and C1's face was cleaned up (sand)). Per S1, C1's parent was called and informed the incident. Per S1, C1 was take to seek medical attention. Per C1, glue and tape was applied to the injured area. LPA informed director that if children are seen leaning on the railing it is a best practice to redirect children to prevent future incidents. There are no deficiencies being cited at this time as the incident appears to be an accident. An exit interview was conducted and a copy of this report was provided to director along with Notice of Site Visit.
