What Happened

Case-management inspection followed up on two incident reports, including a playground fall that resulted in a broken arm and a separate allegation of rough handling by a staff member. The first incident was assessed as fast-moving and not preventable, while the second resulted in a Type B citation.

Observations

  • Child fell from climbing structure in the toddler yard and broke an arm
  • Staff provided first aid and monitored the child on a cot until the parent arrived
  • A separate report alleged a staff member squeezed a child's arm and bounced the child on a cot
  • The second incident was cited as a Type B deficiency

Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting... This requirement was not met as evidence by: Based interviewes C2 stated that S3 would grab and squeeze his arm when c2 was not following directions. This poses a potential health and safety risk to children in care.
