What the inspector wrote

On 10/06/25 at 8:49am Licensing Program Analysts (LPAs) Diana Ortiz and Shushanik Safaryan conducted an unannounced Annual required inspection. LPAs were greeted by Director Dawn Cansler, to whom the purpose of the inspection was announced. LPAs were guided on a tour of the facility indoors and outdoors. This is a preschool age program licensed for 34 children which operates Monday \342\200\223 Friday from 9am- 12pm.The Director stated there are 58 children currently enrolled on various schedules. The facility is located on the same campus as Options for Learning which is shared by the Ramona Acc Christian Church. Preschool is separate from Options for Learning facility. LPA observed a total census of 10 children with 5 staff members in care. All staff members were fingerprint cleared. Licensed facility is within the conditions, limitations, and capacity specified on the license. The following classrooms were inspected: Room 1 (Lavender room) had 3 children in care with 1 staff. Room 2 (Sunflower room) had 5 children in care with 1 staff and one parent volunteer. Room 3 (Daisy room) had 2 children in care with 1 staff and one parent volunteer. Room 3 has a kitchenette which is only used for playing activities such as playdough, and crafts. LPAs observed 1 toilet and 1 sink between Room 2 and Room 3 which is used by all children. LPAs observed a separate restroom which is located at the front of the school and is used by parents and children at drop off. The restroom located in the front of the facility has a changing table which is cleaned and sanitized daily by staff. Currently, the Directors office is used as an isolation area where the children who become ill wait for parents to pick them up. The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Page 1 of 4 Telephone service, heating, lighting, and ventilation were evaluated. Storage for children\342\200\231s belongings was inspected and meet all regulatory requirements. During this time, children being in their own snacks and drinking water from home. Toys and surfaces are observed to be safe and clean. Staff restroom is available near the front of the school. LPAs observed that carbon monoxide and smoke detectors were present in the facility. Fire extinguishers are serviced annually and were last serviced 03/13/25. Storage cabinets in classrooms were inspected, and no hazardous items were found, cleaning items were located on the top shelves of each classroom and inaccessible to children in care. No poisonous cleaning compounds were found in the childcare center. Emergency disaster drill was last conducted on 09/30/25. There is a First Aid kit available inside each classroom; the supplies were inventoried and found to be in good condition. LPAs were informed that there is currently one child on medication and IMS documentation was reviewed and medication inventoried. LPA advised Director of procedures and medication which is currently stored in each classroom and is out of reach of children. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as it is free of hazards. LPAs observed the outside area has adequate age appropriate playing equipment such as play structure, water play activities, tricycles. There is sand all around the play yard, including where the slide is located which can absorb falls. Adult supervision is required when a child is playing with the water. Director states there are no weapons or firearms on the premises. There is adequate shade in the play yard with trees and a patio awning. Sign in and out is done electronically using a parent application \342\200\234Brightwheel\342\200\235. A printed copy of the report was available for LPAs to review. Page 2 of 4 At 10:30am, Licensing staff reviewed 5 staff records and observed that 5 of 5 staff had Heartsaver for K-12 School Child CPR AED, Infant CPR, First Aid which does not meet Title 22 regulations. During today\342\200\231s visit, staff contacted representative who provided training and stated that wrong certificate was issued and will provide correct certificate to staff once they are able to do so. At 11am, LPAs observed children\342\200\231s files reviewed to be complete. Inspection of required forms were made. Children\342\200\231s files reviewed contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPAs did not observe lead testing to be conducted within the timeframe stated above and outlined in PIN 21-21.1-CCP. LPA provided PIN to the facility representative. Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years. Based on the LPAs observations and records review, the following deficiency listed on the attached LIC 809D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. A notice of site visit was given and must remain posted for 30 days.
