What Happened
Case management follow-up inspection found Co-Licensee submitted falsified staff training certificates with inaccurate completion dates and incorrect certificate format.
Observations
- Co-Licensee Randall Graham submitted 4 false mandated reporter certificates dated 2/10/25, 2/11/25, 2/12/25, and 2/13/25
- Certificates printed on old certificate style (vertical layout)
- Certificates show future dates (July 2026)
- LPA previously advised licensee on 2/10/25 that certificates were inaccurate and recommended current training
