What the inspector wrote

On 07/18/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management inspection. LPA met with Gabrielle Alarcon, Director and explained the purpose of the visit. There are 6 children with 2 staff members present today. The purpose of the visit is to follow up on an incident that may have occurred on 06/24/2025. The incident wasn't reported to the facility by a family member until 07/02/2025. The department received the incident report on 07/03/2025. The self reported incident is regarding supervision. During today's inspection, LPA interviewed Staff #1 (S1) in the office and toured the facility with the S1. The facility is not being cited any deficiencies today as the incidents needs further investigation. An exit interview was conducted and a copy of this report was provided to the Director. A Notice of Site Visit was provided; Notice of Site Visit must be posted for 30 days.
