Incident investigation·September 16, 2025

On 09/16/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management visit. LPA met with Gabrielle Alarcon, Director and explained the purpose of the visit. There are 13 children with 3 staff members present today. The purpose of the visit is to deliver finding on an incident that may have occurred on 06/24/2025. The incident wasn't reported to the facility by a family member until 07/02/2025. The department received the incident report on 07/03/2025. The self reported incident is regarding supervision. During the course of the investigation, LPA interviewed Staff #1 (S1) in the office interviewed Parent #1, toured the facility with S1 and reviewed records. Based on interviews, facility tour and record review, there were no corroborating information to determine that there was a lack of supervision. The facility is not being cited any deficiencies today. An exit interview was conducted and a copy of this report and appeal rights was provided to the Director. A Notice of Site Visit was provided; Notice of Site Visit must be posted for 30 days.
