What the inspector wrote

On 03/03/26 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection. The purpose of the inspection was to obtain additional information from the facility regarding an incident reported to the regional office. Upon arrival LPA met with Program Director Rosa Ducoing to whom the reason for the inspection was explained. LPA, who guided LPA on the tour of the facility. Census was taken. LPA observed 30 children with 6 staff. The department received an incident report via email from this facility on 02/20/26 for an incident that occurred at the facility on 02/18/2026 regarding a child sustaining an injury in the play yard. During the inspection LPA conducted interviews with staff and director regarding the incident. LPA obtained documentation in the form of facility roster, ouch report, Doctor\342\200\231s note, and viewed video footage of the incident. Based on the information that was verified during the inspection and the interviews conducted, the incident occurred in the morning during outdoor play in the grass area of the play yard. Child #1 was running alongside child #2, child #1 tripped and fell, stood up and began to walk with a limp. Staff approached child and assessed the situation; an ice pack was applied on the left/right foot. Child\342\200\231s guardian was called and given an \342\200\234ouch report\342\200\235 during the time of release. Child #1 returned to the facility the following day after the injury. Child #1 was taken to seek medical attention on the third day after the injury. Based on the Doctor\342\200\231s note, child#1 was due to return to daycare on 02/23/26 with no restrictions. Page 1 of 2 Based on the information obtained during the investigation, it was determined that there was no violation of California Title 22 regulations at this time. The facility followed all proper procedures. No deficiencies are being cited during today\342\200\231s inspection. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars.
