What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted a Case Management inspection at the above facility to follow up on the self-reported incident that occurred on 2/4/2026. The Monterey Park Southwest Child Care Regional Office received the incident report on 2/4/2026. Upon arrival, LPA was greeted by teacher, Sandra Espino. LPA observed the children were napping. No concerns with the level of care and supervision. About an hour later Center Director, Vanessa San Martin arrived. LPA conducted child's file reviewed and obtained the pertained documents. LPA conducted interviews with staff, child, and other. Based on the facts presented and the information was gathered through interviews. On 02/4/2026, there were 15 children with two staff, a student volunteer, and 5 parents volunteers were present. Incident occurred while the class was at the Farm, C1 was wandering away from the class for about 3 minutes within the perimeter of the farm. When staff conducted head counts and noticed C1 was missing then went looking for C1. At the same time, staff received a call from the farm staff that they found C1. The child appeared to be comfortable. Based on the available information, it appears this incident was the result of a Title 22 violation for lack of care and supervision. 02/10/2026 Center Director had met with the staff and reviewed the safety plan regarding to taking children on field. LPA reviewed the training material that was discussed on 2/10/25. Type B deficiency was cited, and appeal rights provided. Please see 809D. Citation was cleared during today's visit. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and report was reviewed with the facility representative, Vanessa San Martin.

    Responsibility for Providing Care and Supervision-No child(ren) shall be left without the supervision of a teacher at any time,This requirement is not met as evidenced by based on interviews, the facility failed to provide adequate supervision for a child in care. 02/4/2026, C1 wandered away from the group for about 3 minutes then found by the farm staff within the perimeter of farm which poses a potential health and safety risk to children in care.
