At 9:30AM. The Licensing Program Analyst (LPA), T. Tran, conduct an unannounced site visit at Lakeview Head Start to follow up on a Case Management Incident occurred on 11/25/2025 pertaining to a head injury. Upon arrival, LPA met with Family Services Associate Mei Wei. We toured the facility, LPA observed proper care and supervision. About 11:45AM, Education Coordinator, Dominique Pastrana arrival at the site. During today's visit, LPA completed staff and children files review. LPA obtained the child's document. Interviews were conducted with staff, children, and others. Based on information that was gathered through interviews and record reviewed. On the day of the incident, there were 7 children with two teachers. During free choice activity, a peer threw a wooden toy then hit C1 (See LIC 811) on the back of the head. C1 sustained a small cut which required medical attention. Staff immediately provided first aid and contacted the school Nurse and parent. C1 had seemed by the doctor and treated with skin glue. C1 is no longer enrolling at this school. Based on the available information it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further investigation is required for the incident reported. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Dominique Pastrana.
