What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management- incident inspection to the above facility. The purpose of this inspection was to follow-up on an incident that was self- reported to the department on 6/3/2025. LPA met with Program Managers Claudia Casillas and Tommy Velasquez who gave LPA a tour of the facility. Census was taken. On 6/3/2025 an incident was self reported to the department within the required 24 hours. Per Incident reported On 5/30/2025 During a classroom dance activity Child # 1 tripped and fell hitting their nose on a house area furniture (nigh stand) resulting in a nose bleed. LPA conducted interviews with staff, incident was observed- during a dance activity the children were dancing around the classroom. Child # 1 tripped in the house area hitting their nose on a night stand. Child had a nose bleed- staff provided first aid and parents were called. Child was taken to urgent care- diagnosed with a nasal contusion, no broken bones- no restrictions. Child returned to facility on 6/2/2025. At this time the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
