What the inspector wrote

Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced case management inspection at the facility to follow up on an Unusual Incident Report (UIR). LPA met with Mavel Carias, Secretary and informed her of the reason for the visit. There were a total of 41 children with 9 staff. The facility submitted an unusual incident report (UIR) on 11/13/25. Per the UIR, Child 1 was participating in an obstacle course when the child #1 was pulled by child #2 who jumped off of a block that was approximately 15 inches tall. Upon falling off the block when child was pulled, child 1 hit their mouth on child 2's head. Staff 1 witnessed the incident and assisted within seconds. Staff 1 called Staff 2 for assistance. Staff 2 took child #1 to check their wound and took child #1 to the elementary school's nurse's office. The wound was washed and child was picked up within an hour. Facility is requested a medical clearance for the child to return and staff statements. Facility received medical clearance on 11/21/25 and child returned the same day. There were no restrictions. LPA obtained and reviewed documentation. Based on the information obtained and LPA observation of the play area, LPA determined there was adequate supervision provided the day of the incident. Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit. A notice of site visit was provided and must be posted for 30 days. A copy of this report was reviewed and a copy was given to facility representative Mavel Carias, Secretary
