What the inspector wrote

On 3/19/2025 at 9:10 am an unannounced Case Management-Incident inspection was conducted by Licensing Program Analysts (LPAs) Carolyn Tuba and Diana Ortiz to investigate an Unusual Incident Report that was submitted by the facility on 3/11/2025. LPAs entered through the cafeteria of the school as the office is being remodeled. Staff member stated that Child Development Supervisor, Gabriel Munoz was not present, however Director of Child Development, Abigail Querubin-Villareal was subbing in his absence and was in room 26 as it is being used as office space. Director of Child Development guided LPAs on a tour of the facility and observed a census of 153 children with 26 staff in care. The incident was reported to the Department within the required 24 hours of occurrence. The incident that took place was on 3/10/2025 at approximately 4:50 pm and was reported to the Community Care Licensing Department via written Unusual Incident Report (UIR) that during transition time a child was left behind in the classroom. LPAs interviewed Director of Child Development, Staff #1 (S1), and #2 (S2). The Child Development Supervisor, Staff #3 (S3), Parent #1(P1) were not available for interview, however LPAs obtained contact information. LPAs determined that it was not necessary to interview Child #1 (C1). Director of Child Development provided LPAs with statements from staff for their records. During the interview with S1 it was disclosed that during the transition from one classroom into the other classroom at approximately 4:30 pm, S1 disclosed that the initial head count was 7 children, however when taking the children over S1 did not count the children again after delivery those children into the new classroom. S1 stated that S3 who was subbing that day was left behind in the classroom where children were initially and did notice C1 was left behind inside the classroom. S1 stated that according to them, C1 may have hidden inside the classroom as C1 was with the group initially and had been accounted for. page 1 of 2 When LPAs interviewed Child Development Director, S1 and S2, it was confirmed that C1 was left attended in classroom for approximately 20 to 30 minutes based on when S1 left the classroom and when Parent #1 (P1) notified S2 that C1 was alone inside the classroom. The Child Development Supervisor was notified of the incident by S2, and Child Development Supervisor later called C1’s parent according to what was written on the UIR. Both S1 and S2 stated children are counted before transitions, name to face is used and the number of children is written on the board inside the classroom to make sure children are accounted for. The LPAs observed during the census count that when staff and children were coming outside to play, those children were not being counted. LPA, Tuba asked staff member what the procedure was for transitions. Staff member stated that children were counted inside and name to face was also completed before leaving the classroom. LPAs determined that while transitions of counting are in place and name to face is being used, some staff may be missing some of the steps. LPAs also informed Director of Child Development that a meeting will be arranged at a later time with the Department. The following deficiency Lack of Supervision is listed on the attached LIC 809 (deficiency page) and is being cited in accordance with California Code of Regulations Title 22. Deficiency that are being cited need to be cleared to protect the children’s health & safety. One (1) Type A - A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Child Development Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director of Child Development, Abigail Querubin-Villareal. page 2 of 2

Supervision shall include visual observation. This requirement is not met as evidenced by: Based on interviews, and written statements that a child was left unsupervised for 20 to 30 minutes inside the classroom with no adult supervision during transition to another classroom. This poses an immediate Health and Safety risk to children in care.
