What the inspector wrote

On 3/25/2025 at 9:30 am Licensing Program Analyst (LPA), Carolyn Tuba conducted an unannounced Proof of Correction (POC) inspection to ensure the deficiency cited on 3/19/2025, during a Case Management inspection for an incident that was reported to the Department on 3/11/2025 was corrected. A COVID risk assessment was conducted. LPA met with Child Development Supervisor (CDS), Gabriel Munoz. LPA obtained a census of 180 children and 34 staff in care. LPA observed transitions from outside to inside in a variety of classrooms throughout the campus. LPA did observe staff counting and using the name to face, however LPA noted areas that could be improved during transition time and discussed with the CDS while taking the census. LPA was also given a copy of the documents provided to staff (instructions of transitioning) counting children, name to face including signatures of the staff to keep accurate records of who has received the information. CDS also mentioned that a staff meeting will be held next week for additional information. LPA provided the regulation for supervision in a Child Care Center: 101229 Responsibility for Providing Care and Supervision During today’s visit, LPA took the opportunity to interview Staff #3 (S3) who was not present during the initial visit conducted 3/19/2025. S3 confirmed what had occurred during the incident of 3/10/2025 with C1. LPA stated that the CDS and the Child Development Director as well as additional administration will be attending a meeting to be held at the Regional Office of Monterey Park and an appointment time will be set in the near future. page 1 of 2 LPA cleared the deficiency during today’s visit and issued Proof of Correction (POC) clearance letter for the facility’s recordkeeping. At this time, there are no deficiencies being cited and the facility is now in compliance according to Title 22 Rules and Regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Child Development Supervisor (CDS), Gabriel Munoz. page 2 of 2
