What the inspector wrote

On 3/28/2025 at 2:30 pm an unannounced Case Management-Incident inspection was conducted by Licensing Program Analyst (LPA) Carolyn Tuba to investigate an Unusual Incident Report that was submitted by the facility on 3/27/2025. LPA entered through the cafeteria of the school as the office is being remodeled. Staff member stated that Child Development Supervisor, Gabriel Munoz was not present, however Director of Child Development, Abigail Querubin-Villareal was subbing in his absence and was in room 26 as it is being used as office space. Director of Child Development guided LPA on a tour of the facility and observed a census of 109 children with 22 staff in care. The incident was reported to the Department within the required 24 hours of occurrence. The incident that allegedly took place was on 3/27/2025 at approximately 7:40 am and was reported to the Community Care Licensing Department via written Unusual Incident Report (UIR) that Child #1 (C1)'s personal rights were violated. LPA interviewed Director of Child Development, and C1. Due to time restraints additional children and staff were not interviewed at this time. Per Director of Child Development the staff member accused has been placed on administrative leave at this time until further investigation is conducted internally and incident was also reported to Department of Child and Family Services Director of Child Development provided LPA with staff statements, schedules of the staff that require to be interviewed, parent contact information, as well as the children present during morning drop-off. LPA will return to conduct further investigation of the incident. No citations were issued during this visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director of Child Development, Abigail Querubin-Villareal.
