On 4/2/2025 at 09:05 am an unannounced Case Management - Incident inspection was conducted by Licensing Program Analysts (LPAs) Carolyn Tuba and Monica Ruiz to investigate an Unusual Incident Report that was submitted by the facility on 3/27/2025. LPAs entered through the cafeteria of the school as the office is being remodeled. Staff member stated that Child Development Supervisor, Gabriel Munoz was in room 26 during the remodel of the office. Child Development Supervisor guided LPAs on a tour of the facility and observed a census of 188 children with 29 staff in care. The incident was reported to the Department within the required 24 hours of occurrence. The incident that allegedly took place was on 3/27/2025 at approximately 7:40 am and was reported to the Community Care Licensing Department via written Unusual Incident Report (UIR) that Child #1 (C1)'s personal rights were violated. LPAs spoke to the Child Development Supervisor and interviewed Staff #1 (S1), #2 (S2), #3 (S3) and #4 (S4). Due to time restraints additional children and staff were not interviewed at this time. LPAs were also provided a facility roster. LPAs will return to conduct further investigation of the incident. During the investigative visit Child Development Supervisor disclosed that the Sheriff's Department had conducted a visit and contact information was provided. No citations were issued during this visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Child Development Supervisor, Gabriel Munoz.
