What the inspector wrote

On 4/28/2025 at 10:30 am a follow-up unannounced Case Management - Incident inspection was conducted by Licensing Program Analyst (LPA) Carolyn Tuba to investigate an Unusual Incident Report that was submitted by the facility on 3/27/2025. LPA entered through the cafeteria/multi media room of the preschool as the office continues to be remodeled. Staff member stated that Child Development Supervisor (CDS), Gabriel Munoz was in room 26 during the remodel of the office. Child Development Supervisor guided LPA on a tour of the facility and observed a census of 187 children with 20 staff in care during nap time. The incident was reported to the Department within the required 24 hours of occurrence. The incident that allegedly took place was on 3/27/2025 at approximately 7:40 am and was reported to the Community Care Licensing Department via written Unusual Incident Report (UIR) was that Child #1 (C1)'s personal rights were violated. LPA interviewed Staff #6 (S6) and Child #2 (C2) and #3 (C3) during today's visit. On a separate occasion, LPA conducted a collateral visit and interviewed Staff #5 (S5). During the visit LPA and CDS discussed the Technical Support Program (TSP) that is offered through the Department and having TSP consult with the entire staff about Care and Supervision and Personal Rights of a child. LPA will refer the facility and TSP would then contact the CDS to set-up a consultation meeting. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Child Development Supervisor, Gabriel Munoz.
