Licensing Program Analysts (LPA) Cynthia Reyes conducted a case management inspection due to an incident that occurred on 12/18/2025. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Program Director Abigail Querubin-Villareal, who the incident was gone over with. The facility self reported that a 3 year old child was left in his class room alone for about 5 to 8 minutes. LPA requested documents regarding the date of the incident to review. The facility also provided the LPA with copies of statements by staff involved, emails and copies of the name to face log for that date. LPA conducted interview with a staff member however due to time restraints additional staff were not interviewed at this time. LPA will return to conduct further investigation of the incident. No citations were issued during this visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Abigail Querubin-Villareal Program Director .
