What the inspector wrote

Licensing Program Analysts (LPA) Cynthia Reyes conducted a case management inspection due to an incident that occurred on 12/18/2025. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Denise Villacres- acting child development supervisor, who the incident was gone over with and census of children and staff were taken for this date. Abigail Querubin-Villareal Program Director arrived later during the visit. The facility self reported that a 3 year old child was left in his class room alone for about 5 to 8 minutes. Per interview with staff it was stated that child #1 was fond alone in a class room by a clerical staff while the other children were using the bathrooms located outside. Per interviews the child could have been alone for about 5 to 30 minutes. It is unknown if the child return to the class room during or after bathroom time. For the incident report the facility followed all the proper procedures per the parent hand book/admission agreement, incident reports were called and sent in properly and timely, all medical needs were met if needed and parents notified, however based on the Lack of care and supervision, the teaching staff were not a wear of the child being left alone in the class room, the facility is being cited. Based on interviews conducted and records received and reviewed, the preponderance of evidence standard has been met, therefore the above incident of Responsibility for Providing Care and Supervision did occur. California Code of Regulations, Title 22, Division 12 & Chapter 1, are being cited on the attached LIC 809D page. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. Page 2 A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100 Exit interview conducted and report was reviewed with Abigail Querubin-Villareal Program Director LPA, Cynthia Reyes informed Abigail Querubin-Villareal Program Director, that this report dated 03/12/2026 document(s) (2) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety or personal rights of a child in care. Also, LPA, Reyes informed Licensee, to provide a copy of this licensing report dated 03/12/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or written statement, must be placed in the child’s file for verification

This requirement is not met as evidenced by: Per interviews and documents received, it was confirmed that there were 3 CDT (child development technicians). left alone in a classroom with 14 children and no qualified teacher on 12/18/2026.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced by: Per interviews on 12/18/2026 a child was found in a class room alone for about 5-30 minutes (Exact time is unknown). This poses an immediate health and safety risk to children in care.
